Non-Operative Options

Most spinal conditions are managed without surgery. This is an overview of the conservative options that are usually tried first.

Surgery is the exception, not the rule. For most people with back or neck problems, the right first step is a course of well-organised non-surgical care.

The great majority of spinal conditions — including most back pain, neck pain, sciatica and early degenerative change — improve without an operation. Conservative care is not “doing nothing”: it is an active, structured plan, and for many people it is all that is ever needed.

Staying active & activity modification

Prolonged bed rest is generally unhelpful and can slow recovery. Keeping gently active within the limits of comfort, while temporarily avoiding the specific movements that aggravate symptoms, is usually advised. The aim is to keep moving without flaring the problem.

Physiotherapy & exercise

A tailored programme of stretching, core and postural strengthening, and graded exercise is a cornerstone of non-surgical care. A physiotherapist can also guide technique, pacing and a safe return to activity. Specific exercise approaches exist for particular conditions, such as scoliosis.

Medication for symptom relief

Simple pain-relieving and anti-inflammatory medication, used appropriately and for a limited period, can make it easier to stay active while the problem settles. Medication choices should be discussed with a clinician, particularly if you have other health conditions.

Injections

Image-guided spinal injections — such as nerve root blocks, epidural injections and facet joint blocks — can reduce inflammation around an irritated nerve or joint. They are used both to relieve symptoms and, sometimes, to help identify the source of pain, and they fit within a broader non-surgical plan rather than replacing it.

Bone & general health

For conditions such as osteoporosis, treatment of the underlying bone health — through medication, calcium and vitamin D, and weight-bearing activity where appropriate — is central. General measures such as not smoking, maintaining a healthy weight and good overall fitness support spinal health too.

When non-operative care is not enough

Surgery is considered when conservative measures have not relieved a problem that is significantly affecting life, or when there is a specific reason not to wait — such as progressive nerve compression, instability, or red-flag features. Even then, the decision is individual, weighing the likely benefit against the risks.
